Using the Accreditation Application Form

The Accreditation Application form is available in PDF format. Numbers will be calculated for you where applicable. The form should be saved to your computer’s hard drive (instructions below) to best utilize Acrobat's features. Completed application forms can be submitted to ECFA along with supporting data and an application fee of $500 to:

How to complete the accreditation application:

STEP 1: IMPORTANT: SAVE THE APPLICATION PDF FILE TO YOUR COMPUTER

Saving the file, Method 1 (Mac/Windows)

  1. Click on the Application Form link below. This will cause Acrobat to open the file in your browser. At this point, USE THE ACROBAT MENU BAR (not the web browser menu). Click on the "disk" icon to save the file.

  2. Choose the location where you want to save the file and click Save.

  3. Close or minimize your browser window. Go to Step 2: Open the file with Adobe Acrobat, below.

Saving the file, Method 2 (Windows only)

  1. Right Click (Windows) on the Application Form link below. In Internet Explorer, choose Save Target As... or in Netscape, choose Save Link As...

  2. Choose the location where you want to save the file and click Save.

  3. Close or minimize your browser window.

STEP 2: Open the file with Adobe Acrobat

Double click the "apply.pdf" file from the location where you saved it. This will open the file in Acrobat.

Note: If you do not have Adobe Acrobat 4.0 (or higher), click here and follow the instructions for downloading.

STEP 3: Fill in the required information

  1. Select the hand tool.

  2. Position the pointer inside a form field, and click. The I-beam pointer allows you to type text. The arrow pointer allows you to select a button, a check box, a radio button, or an item from a list.

  3. After entering text or selecting an item, check box, or radio button, do one of the following:

    • Press Tab to accept the form field change and go to the next form field.

    • Press Shift + Tab to accept the form field change and go to the previous form field.

    • Press Enter (Windows) or Return (Mac OS) to accept the form field change and deselect the current form field. In a multi-line text form field, Enter or Return goes to the next line in the same form field. (You can use Enter on the keypad to accept a change and deselect the current form field.)

  4. Other information:

    • Press Escape to reject the form field change and deselect the current form field. If you are in Full Screen mode, pressing Escape a second time causes you to exit Full Screen mode.

    • To undo changes you have entered:

      1. Select Edit > Undo. Keyboard shortcuts: Ctrl + Z (Windows) or Command + Z (Mac)

    • PLEASE NOTE: If you are attempting to fill out the form in your browser, clicking the Reload button, or the Go Back button, or following a link in a Web browser window DOES NOT CLEAR a form. This is why we recommend saving the file and opening it in Adobe Acrobat, instead of a browser.

STEP 4: Print out the completed form.

STEP 5: Sign all signature fields.

STEP 6: Attach all supporting documentation.

STEP 7: Mail the form and all documents to ECFA.
